
FERPA Notification and Consent for Release of Records
To comply with the Family Educational Rights and Privacy Act of 1974 (FERPA), Reid State provides a printed form in Student Services and a digital form through the college application for students to authorize the Enrollment Specialist–Admissions (or designee) to share confidential information and educational records with individuals or agencies specified by the student.
Educational records, as defined by FERPA, include records, files, documents, and other materials directly related to the student and maintained by the College or its representatives.
This form allows students to grant permission for others—such as a parent, guardian, or spouse—to discuss their academic records. Students are informed of their FERPA rights annually in full compliance with the law.
To revoke consent, a student must submit a signed letter indicating the individual(s) affected and send or deliver the notice to:

Students also have the right to file a written complaint with the U.S. Department of Education regarding the College’s compliance with FERPA. Complaints should be sent to:
